IMPORTANT: Appeal Submissions must be filed no later than 4:00 p.m. on the 10th working day after the date of the Decision made by the Decision-Maker.
A Notice of Appeal will not proceed for review to the Undergraduate Academic Appeals Committee (Appeals Committee) if the Appeal Submission is substantively incomplete. Before completing this form, please make sure you carefully review the Undergraduate Academic Appeals Policy (“Policy”) and Undergraduate Academic Appeals Procedures (“Procedures”).
If you have any questions or need help in this Appeal process, please contact your academic advisor. The Ontario Tech Student Union can also assist you in navigating this process.
NOTE: Students must have exhausted all formal processes at the Faculty and/or Office of the Registrar’s level before filing an appeal with the APPEALS COMMITTEE.
Under the Procedures, the time limits specified may be extended by the APPEALS COMMITTEE Chair at the request of the student if reasonable grounds are shown for the extension. Please note that if reasonable grounds are not shown, your request for a timeline extension will be denied, and your Appeal will not proceed for review under the Procedures.
