Please submit one nomination declaration per position. Where multiple positions are available in a particular constituency, a candidate can only be nominated for one position.

Individuals can only stand for election or nominate a candidate from within their own constituency group.

This completed and signed declaration nominates the candidate, and confirms their eligibility and acceptance of the terms and conditions of the nomination.
