What?
This form is used for students seeking to defer a final examination. A student requesting a deferred final examination must:
1. Contact course instructor directly
2. Submit the Academic Consideration form, along with supporting documentation and/or Personal Attestation form
3. Submit this form; Application for Final Exam Deferral, for each exam deferral request
If the deferral reason is medical, a University Medical Statement must be uploaded as supporting documentation. We do not accept regular doctor notes. We reserve the right to verify the authenticity of any health certificates submitted.
Who?
This form must be completed by the student.
When?
-Completed applications must be submitted no later than 3 business days after the scheduled examination date along with a completed Academic Consideration form.
-For religious observances and competing academic conflicts, the application must be submitted no later than 15 business days prior to the first day of the Examination Period - Fall 2024 deadline: November 13, 2024
The processing time for deferral decisions can take 5-10 business days. Deferral decisions are always at the discretion of the university. Deferral decisions will be emailed to the student’s ontariotechu.net account.
Please note: If you are granted an examination deferral and did not write the deferred exam on the scheduled date, a grade of zero will be recorded for the final examination.

The Academic Consideration form is located in your MyOntarioTech account, under Current Students, under the Undergraduate Forms box.
Medical: University Medical Statement must be uploaded as supporting documentation. They must be completed within 24 hours of the missed exam.

Fee
A $45 non-refundable, per exam, administrative fee will be added to your tuition account only after you receive an approval notification.

Please note:
- Charges will be applied only after a final exam deferral is approved. You will be notified via email with instructions on when and how to pay.
- If you are submitting this request due to bereavement or religious observances, you will not be charged the administrative fee.
- If you are required to reschedule your exams as part of an accommodation plan, you will not be charged the administrative fee.
- The rule that indicates you cannot write three exams within 24 hours does not apply to deferred exams.
