DTAP payment confirmation will be sent to the dependent student's email.
The DTAP payment will reflect the mailing address entered.
The name of the post-secondary institution must be written in full (e.g. University of Ontario Institute of Technology). Please note that only Ontario post-secondary institutions are eligible.
Please scan and save Proof of Payment to your files. Then, select the saved Proof of Payment file and upload. Proof of Payment must accompany the DTAP application.
I have read and understood the Dependent Tuition Assistance Program and certify that I am an eligible employee under the terms of the Dependent Tuition Assistance Program. I also understand that the benefit, if applicable, will be added to my dependent's taxable income in the year the reimbursement is distributed.
