Request for Ergonomic Assessment Form

Ergonomic assessments can assist in identifying risks associated with improper equipment use, repetitive tasks and poor workstation set-up. Ontario Tech University’s Environmental Health and Safety Officer can recommend changes to your workstation which can be taken into consideration when working on-campus or from your home office.

Before submitting this request for assessment, please complete Steps 1-3 on the Ergonomics Program webpage and inform your manager that you are submitting a request for assessment.
