This section may be used to notify the university of a legal name change.

Note: Official documentation must accompany this form to verify the legality of any change to your name (e.g. official change of name document, birth certificate or marriage certificate). One form of government-issued photo ID must also be presented (e.g. driver's license or passport). Ensure all forms of ID support the name change before making your request. If nothing is indicated in the middle name field and we currently have a middle name on file, the middle name on file will be removed.

Legal gender designation change

This section may be used to notify the university of a legal gender designation change.

Note: Official documentation must accompany this form to verify the legality of any change to your gender designation (e.g. birth certificate/registration). One form of government-issued photo ID must also be presented (e.g. driver's license or passport). Ensure all forms of ID support the name change before making your request.

Upon completion of a Gender Designation change, all records will be updated to reflect this change, and a notice will be sent to applicable staff on a need-to-know basis to ensure recognition of the change.

Chosen first name

The university recognizes that you may prefer to be addressed by a first name other than your legal first name. For this reason, the university permits students to designate a chosen first name that may be used wherever possible in the university systems throughout their study period. A preferred name can currently be viewed on:
- Canvas
- Class lists
- Ucard
- Alumni Card

Documentation is not required for this request; however, you must submit a copy of your student ID card when submitting this form.
