Short-term Disability Post-return-to-work Feedback Form

As a manager who has recently supported an employee through the various phases of our short-term disability program (i.e. before leave, during leave, return to work), you are being asked to complete this short survey. Feedback received will help the Health and Disability Management Specialist to identify where further support may beneficial and contribute towards a sustainable return to work.
