Use this form if you wish to be considered for late withdrawal from a course or program. Such requests must be submitted no later than 10 working days after the start of classes for the subsequent academic semester. The length of time for requests to be reviewed is normally three weeks. During peak times, processing time may be longer.
Do not use this form. Graduate level students must fill out the Tuition/Late Withdrawal Appeal form instead.
