Ontario Tech University’s Office of Equity, Diversity and Inclusion is conducting this survey. Your participation is important. The information collected in this survey will support the university's efforts to ensure its workforce and community reflect the diversity of Canada, Ontario and the Durham Region. This information will inform implementation, evaluation and monitoring of employment equity measures to improve the recruitment, engagement, retention, success and support of current and future employees, students and community members.

Why self-identify?
As this is the first time Ontario Tech has asked faculty, staff and students to share identity-focused information with the university, we understand there may be some hesitation or concern about disclosing certain aspects of one's identity. One of the university’s strategic priorities is to ensure a learning and working environment where everyone can thrive and feel like they belong; a key part of measuring how we are doing in this area is knowing who is in our community and how they identify. Although this survey is not anonymous, we will keep responses in the strictest confidence (see below), and only a limited number of staff in the university’s Office of Institutional Research and Analysis (OIRA) will be able to link data for analytical purposes. This will allow us to understand the makeup of our university community, and determine the composition of faculties, departments, employment groups, the student body, etc. You will be able to update your responses at any time and choose which questions you want to answer.

Privacy and confidentiality
Protecting the privacy and confidentiality of employees and students is crucial in this process. The university will engage in a series of best practices to ensure that the privacy and confidentiality of survey participants is maintained at every step of the process. Personal information in this survey is collected under the authority of the University of Ontario Institute of Technology Act, SO 2002, c. 8, Sch. O. At all times personal information provided in this survey will be collected, protected, used, disclosed and retained in compliance with Ontario’s Freedom of Information and Protection of Privacy Act R.S.O. 1990, c. F.31.
Although this survey is not anonymous, we will keep responses in the strictest confidence and use only for the purposes outlined above. Only a limited number of staff in the Office of Institutional Research and Analysis (OIRA) who are subject to appropriate privacy training and confidentiality obligations will be able to access the data to support the university’s work to link data for analytical purposes.

Participation
Your participation is voluntary. You are welcome to select 'I choose not to complete this survey' on the next page. This will allow the university to include you in the response rate even if you did not choose to provide any information. If you choose to participate but do not want to answer particular questions, each question allows for you to select 'I choose not to answer this question' as a response. The survey will take less than 5 minutes to complete.
