Personal information on this form is collected under the authority of the University of Ontario Institute of Technology Act, SO 2002, c. 8, Sch. O. and will be collected, protected, used, disclosed and retained in compliance with Ontario’s Freedom of Information and Protection of Privacy Act R.S.O. 1990, c. F.31. This information will be used to administer the public engagement, and the public information and disclosure programs for the Subcritical Assembly Project. The Principles of Ownership, Control, Access, and Possession is used with all data collected regarding Indigenous community engagement and we respect the community privacy of all Indigenous sovereign nations. We encourage Indigenous persons providing feedback to let us know so that we are aware of the data being collected and can apply the appropriate data management practices.
