Fill out this form if you are an undergraduate student requesting to drop a course and are unable to do so online through MyOntarioTech. This form can only be submitted during the deadlines outlined in the Important Dates section of the Academic Calendar. The date of submission will always be the determining factor for the type of drop processed. Please note: Graduate level students must fill out the Graduate Studies - Add/drop courses form instead.
