Code of Conduct *

I/we have read, understood and agreed to the 2025 Ontario Tech University Child & Youth Programming Code of Conduct. I/we warrant that I/we am/are the parent(s) or legal guardian(s) of the individual named in the online registration form who is a minor and that I/we am/are entitled to sign this Code of Conduct. I/we indemnify Ontario Tech University, and its successors and assigns against any loss or claim if the minor disavows the Code of Conduct because the minor was a minor or for any other similar grounds.

Photo/Video Release *

I/we have read, understood and agreed to the 2025 Ontario Tech University Child & Youth Programming Photo/Video Release. I/we warrant that I/we am/are the parent(s) or legal guardian(s) of the individual named in the online registration form who is a minor and that I/we am/are entitled to sign this release form and grant the rights granted therein to Ontario Tech University. I/we indemnify Ontario Tech University, and its successors and assigns against any loss or claim if the minor disavows the release form because the minor was a minor or for any other similar grounds.
Opt Out
I/we warrant that I/we am/are the parent(s) or legal guardian(s) of the individual named in the online registration form who is a minor and do not consent to the use of any image or likeness captured and recorded for the purposes stated in the 2025 Ontario Tech University Child & Youth Programming Photo/Video Release.

Durham First Aid Inc. Release and Waiver of Liability and Indemnity Agreement *

I/we have read, understood and agreed to the Durham First Aid Inc. Release and Waiver of Liability and Indemnity Agreement. I/we warrant that I/we am/are the parent(s) or legal guardian(s) of the individual named in the online registration form who is a minor and that I/we am/are entitled to sign the Durham First Aid Inc. Release and Waiver of Liability and Indemnity Agreement. I/we understand and agree that agreeing to the Durham First Aid Inc. Release and Waiver of Liability and Indemnity Agreement during course registration is the equivalent to providing my written signature.

Cancellations and Refunds *

Paid Camp/Course Cancellations

Cancellations for paid camps/courses made in writing to summercamps@ontariotechu.ca at least twelve (12) days prior to the start date of the related camp/course are entitled to a partial refund. A cancellation fee will apply. Refunds will not be given for cancellations made after this date.

In-person Summer Camp	$40 (per camper, per camp)
In-person Youth Safety Course	$20 (per registrant, per course)
Virtual	$20 (per camper, per camp)

Campers who do not attend their registered camp/course without a notice of cancellation are considered a No Show. In this case, refunds will not be given.

A camp/course may be cancelled if the minimum number of participants required to run the camp/course is not met. Early registration is recommended to avoid cancellation due to low registration. Note: Where cancellation of a camp/course occurs due to low registration, registrants of the affected camp/course will be given a full refund.

Free/Scholarship Camp Cancellations

To ensure camps/courses remain available to interested campers, we request that cancellations for a free camp/course or scholarship funded camp/course registration be submitted to summercamps@ontariotechu.ca at least twelve (12) days prior to the start date of the related camp. Because of limited availability, please only register if your camper plans to attend all sessions. Cancellation fees do not apply to a free camp/course or scholarship funded camp/course registration.

Assumption of Risk and Release of Liability *

I/we have read, understood and agreed to the 2025 Ontario Tech University Child & Youth Programming Assumption of Risk and Release of Liability. I/we warrant that I/we am/are the parent(s) or legal guardian(s) of the individual named in the online registration form who is a minor and that I/we am/are entitled to sign this Assumption of Risk and Release of Liability. I/we indemnify Ontario Tech University, and its successors and assigns against any loss or claim if the minor disavows the Assumption of Risk and Release of Liability because the minor was a minor or for any other similar grounds.

Policies *

I/we have read, understand and agree to the camp policies listed on the 2025 Ontario Tech University Child & Youth Programming policies page, including but not limited to: general health and safety, online discussion rules, supervision and support, technology and material requirements, registration and payment, cancellation and refunds, waitlists, and photo/video release.
