Emergency camper information

Ontario Tech University is committed to providing a safe environment for all children attending camps at the University. The following information is mandatory and will remain confidential. This information will only be used as necessary by University staff, campus security, police or other law enforcement and/or emergency services agents in the event of an incident. Camper photos may be used by University staff to verify photo/video consent, where necessary.

Medical information

These include, but are not limited to, life threatening allergies and/or conditions that affect your child's level of participation. Please include any seasonal allergies or pre-existing conditions that are not COVID-related (e.g. chronic runny nose, congestion, migraines).

Parents/guardians will be required to contact the Campus Health and Wellness Centre to make arrangements for a health care professional to administer the prescribed medication(s). An appointment prior to camp may be required. See 2026 Ontario Tech University Child & Youth Programming policies page for details. Please note: Camp staff will not administer, supervise the administration of, or store prescribed medication(s).
Medical intervention waiver
In case of emergency or illness, every effort will be made to contact the parent/guardian. In the event that the parent/guardian cannot be reached or the nature of the exigent circumstances do not allow for such contact to occur, a qualified health professional or medical attendant may attend to the Participant and administer appropriate treatment, including hospitalization, if required.
If Ontario Tech University, at the direction of a qualified health professional or medical attendant, sends the Participant directly to the nearest hospital by ambulance, the parent/guardian listed at the time of registration will be solely responsible to pay for any ambulance services. *
